Home Visit

Do you allow a representative to complete a home visit?

• A safe collar with rabies and I.D. tags (provided) to be worn at all times.

The dog will be provided with adequate fresh food and water, clean, dry shelter when outside, and daily exercise.

The Foster Family agrees to foster the following dog (hereinafter called, "Dog") from Southeast Shih-Tzu Rescue and Hospice (hereinafter called “SESTR").
